Description:

The Community Health Worker/Promotor de Salud promotes health within a community by assisting individuals to adopt healthy behaviors. CHW’s partner with families and honors diverse family structures and parenting practices. Activities may also include but are not limited to administering the Family Resilience and Opportunities for Growth Scale (FROG) Scale; administering screening tools such as the ASQ, ASQ-SE, Depression Screens etc.; identifying and referring families for other supportive services, including health care services. The CHW will also be responsible for assisting the family in establishing goals and supporting them throughout this process and is responsible for implementing activities outlined on the Family Service Plan to address areas of concern that were identified on the FROG scale. Serve as an advocate for the health needs of individuals by assisting community members in effectively communicating with healthcare providers or social service agencies. Completion of outreach activities including acceptance of referrals from clinics and other social-service organizations, door-to-door contacts, health, and community fairs.

Minimum requirements:

  • High School Diploma or equivalent req; 2-3+ years of related/equivalent experience in a similar position with training/certifications; 80hrs.
  • Bilingual (Spanish/ English; speak and write) preferred.
  • Receive clearance from an FBI background check (Custodian of Records for DOJ).

Qualifications:

  • Minimum of 2 years of experience as a CHW, Promotor de Salud, or similar role.
  • Willing to commute within the surrounding work area.
  • Strong knowledge of public health principles and community resources.
  • Excellent interpersonal, communication, and organizational skills.
  • Proficiency in outreach methods, including door-to-door engagement and event participation.
  • Basic knowledge of Microsoft Office Suite and data collection tools.
  • Ability to work independently and as part of a team.
  • A desire for frequent local travel to meet with participants and attend community events.
  • Proven ability to multitask, prioritize tasks, and manage time effectively.
  • Capable of working/ understanding the needs of a multicultural environment.
  • Strong work ethics, maintaining a professional appearance, and exercising discretion with sensitive information.
  • Ability to perform tasks related to physical activity to include sitting, standing, for long periods of time, caring, bending, moving, and lifting up to 50 lbs.

Responsibilities:

  • Serve as a bridge between the community and healthcare providers, ensuring individuals’ voices are heard and needs are met.
  • Advocate for equitable access to healthcare and social services for underserved populations.
  • Build trust and foster strong relationships with community members, healthcare organizations, and social service providers.
  • Provide culturally relevant health education on topics such as chronic disease prevention, nutrition, mental health and various other facets.
  • Assist individuals in developing personalized strategies to adopt and sustain healthy behaviors.
  • Educate community members about available healthcare and social resources, guiding them in navigating systems effectively.
  • Implement health initiatives that promote, maintain, and improve individual and community health outcomes.
  • Conduct door-to-door outreach, attend community fairs, and collaborate with local clinics and organizations to connect with individuals in need.
  • Deliver preventive services, such as health screenings and vaccination education, in collaboration with healthcare teams.
  • Collect and report data on community health trends, outreach efforts, and program effectiveness.
  • Assist in identifying emerging health needs within the community to guide program development.
  • Accept and manage referrals from healthcare providers, social-service organizations, and other partners.
  • Facilitate access to medical appointments, social services, and other resources, ensuring follow-through and continuity of care.
  • Support and mentor other Community Health Workers, providing guidance and sharing best practices.
  • Participate in the training and onboarding of new CHWs to maintain a high standard of community engagement.
  • Help organize and participate in health education workshops, community meetings, and public health campaigns.
  • Represent El Sol NEC at health-related events, promoting organizational programs and services.
  • Maintain accurate and confidential participant records, ensuring compliance with organizational and regulatory standards.
  • Participates in staff meetings, individual supervision, and training related to their work.
  • Other duties as assigned.
